Fallbrook partners with Houston Methodist Hospital to invite you to receive the COVID-19 vaccine if you are 50 and older. You must be 50 years or older to be eligible for scheduling. If you fill out this form, Houston Methodist will soon contact you via text message to help you schedule a vaccination appointment. In the text, you will receive a contact number if you need assistance with scheduling. If you do not have a mobile number, you will be contacted at your home number to schedule. If needed, Houston Methodist can also assist with transportation. Thank you for joining our fight to stop the spread of this virus. Vaccination will take place at a Houston Methodist Facility. |
